Winifred Childress Memorial Scholarship
The members of the Royal Oak Lions Club are proud to provide scholarships for post secondary education.
Scholarships will only be available to residents of the Royal Oak School District and/or those who have graduated from a school located in the city of Royal Oak.
The Committee may award one or more scholarships in amounts of at least $500. (All checks for scholarships will name both the school and the student awarded the scholarship.)
Selection Requirements
1. The candidate must be Royal Oak high school graduate or a resident of Royal Oak.
2. Priority will be given to a candidate who is hearing or vision impaired, or diabetic or who is pursuing a career
in health and human services.
3. Economic need of the candidate shall be considered in the case where multiple candidates qualify
for the scholarship.
4. Applicants must have a 2.5 GPA or above.
5. Candidates must have significant community service
To apply for the Scholarship, please complete the application below and provide the following information:
¨ Completed scholarship application form for 2006
¨ An essay - a typewritten statement of no more than three double-spaced pages describing your educational and personal goals, your work experience, extra-curricular activities, and a statement about how the scholarship award will be used.
¨ Official transcripts - an official original transcript from the school you are presently attending or most recently attended. Undergraduates please include high school transcript(s); graduate students please include undergraduate transcript(s).
¨ Proof of post-secondary acceptance - a notice or letter from a college or university admissions office documenting that you have been accepted to post-secondary studies, an undergraduate, or graduate college or university.
¨ Two (2) letters of recommendation - from an academic advisor, teacher, or community leader.
¨ Proof of legal blindness - a statement prepared by an ophthalmologist or optometrist certifying that you are legally blind (i.e., have a best corrected visual acuity of 20/200 or less in the better eye, and/or a visual field of less than 20 degrees). Or official letter from a state or private agency for the blind. If applicable.
¨ Proof of U.S. citizenship - a photocopy of a birth certificate, naturalization papers, or a U.S. passport.
